Only exception is some menu states in some games. They have ”automatic fps limit”. Bg3 menus do not have this. So you GPU needs to go near 100% in menus also.
If you want less than 100% you need to set FPS limit manually. Which you now did.
I use my GPU always 100% in modern games. And I only buy GPUs that can handle full 100% without issues or too much noise.
You have really garbage GPU model if 100% load means 100% max fan speed. With my Asus 3080 TUF 100% load only means 65 celcius and about 50% fan speed.
Or you gpu cooler contact etc has gone bad and you have warranty case.
Or you have managed to mess up your gpu fan control setting with too many controlling software (radeon own, afterburner etc all controlling).
The point is that this is an unreasonable amount of gpu load. some horrible optimisation error. Do you not see 99% gpu use on the main menu as an issue? it's obvious that there is some other issue at fault.
Or if we concentrate on one single and same game. Same game with ultra settings can give fps 80 and load 100% or very low settings 300 fps and load 100%. Again load only reduces if you use FPS limit of lower value than the fps is.
Bg3 main menu has moving living cave in background that is heavy graphics and will consume 100% as it should. Only way it does not consume 100% is FPS limit set by game itself (some games have decided that main menu can be automatically limited because it is not important to game experience or it is very light text graphics and fps would be 1000 without limit). Or fps limit set by you.
I play this game 4k ultra settings with fps limited to 120 (maximum fps my TV supports). But this game like many many modern games 4k resolution does not give nearly as high as 120 fps and limit is not hit and load is 100% all the time. I use same 120 fps limit for all games. So if I play some 10 years old game with light graphics that could give 600 fps at 100% load it hits to 120 fps limit and GPU load is 20% as result.
Generally this game is not in final optimization quality. But it is still reasonably normal on higher level setting. What I have read and partially tested my self is that lower setting levels might not be as well optimized. They actually do not make things lighter and raise fps that much. I have tested this and I use maxed out ultra settings always now. They work well and give decent FPS still.
Also the newest added settings like nvidia DLSS and AMD FSR might have problems. I have seen some complaints, but I have not tested.

I was happy playing this game on a gtx 1060, but now I have a worse experience. despite having a far better gpu by a whole magnitude, if not more.
So you are missing the point also. I will quote myself also. Your ”maxed out RPM” is only your personal problem. Everyone else can run this 100% load without problems:

But it is absolutely irrelevant. It will not effect gaming experience of anyone. Except yours because of "my GPU fans max out in RPM" I guess.
So it might or might not get optimized. On my list it's bottom.
But if you are not then similar issue was fixed by using older AMD driver.
Seems that AMD is back in old tricks and having lots and lots of driver issues with new 7000-series and messing up old cards at same time.
