First, do this from the Larian multiplayer issues FAQ:
"If you can not connect to anyone else, try starting the game and switch to windowed display mode and lower the game resolution. Then from the main menu click on Multiplayer, switch to the LAN tab, and create a lobby. In the Lobby Settings on the right, make sure LAN connections are enabled. Next, start another instance of the game directly from the bg3.exe or bg3_dx11.exe executable, in the '..\SteamApps\common\Baldurs Gate 3\bin' folder, then when it loads select Multiplayer, switch to the LAN tab, and try joining. With only the local networking involved and identical data, that should work."
This "works" in the sense that I now have two copies of me in my own lobby.
Incidentally, one has my Steam profile icon (the default question mark) and the other is blank.
Now, quit the lobby from the SECOND instance of the game and create a new lobby. Send a steam invite from the SECOND lobby to your friend. For us, she was able to join! Then just close down the first instance of the game.
Interesting as I tried on my end earlier and it did not work, will give it another shot.

What I realized was that on both PCs we were using my same Microsoft account and all the files (perhaps most importantly our steam user data) were being saved/synced across the two PCs. My fix was that I created a separate user account on the PC for my wife and voila! works normally. Hope this helps!
Larian needs to fix their game.
- baldurs gate launcher
- option
- open profile folder
- click player profiles
- delete all items in playerprofiles
- save changes
- relaunch the game
