If you have disadvantage the game rolls two d20 to hit and pick the lowest instead of just one d20. If you have an advantage the game rolls two d20 and pick the highest.
Str bonus makes it easier to hit with melee, a finesse weapon can also use dex to hit if that is better than yor strenght. Astarion for example should use a weapon like the rapier that have the finesse tag.
Dex is also used for ranged attacks.
Spells use your spell attack bonus, so depends on class, either int, wis or cha.
Bless gives you a bonus to hit, Fairy Fire makes the cratures hit easier to hit.
Prone or paralyzed enemies are easier to hit. e.t.c.....
You can hover the mouse over for example the damage summary in your inventory and see the bonuses.
You also need to be proficient with a weapon to get the proficency bonus that goes up with your levels.
