As for the realism argument: It's also realistic to have the characters relieving themselves but I don't see the need. Not everything realistic is desirable or necessary.
But each to his own...
Speaking of that (half offtopic related) : On Assassin's Creed Valhalla , did you know? in that side mission which includes nudist colony , those undressed characters are censored behind censorgarments yet that character says ''we are nudist'' but why they still wearing?... also yet even with Nudity option toggle there is ON and yet they are still censored behind censorgarments.
How and such Disgrace , Ubisoft dont know or tthey trying to make fun or false info on Nudist/Naturism life or somthing like that
So on this game ,yeah i say those npcs in garments could make sense if the character or the story context mentions they found this garments piece, but if the context or character voice mentions naked and yet/while naked npc wearing garments then could be censorship related ( like on ACV nudist censorship is ) and should have uncensor option for that.

And Also, no one said BG3 is a survival game.
Look at Elder scroll 1,2 , Mortal online (and its upcoming sequel) etc, they are not survival games and it has realistic undressed to actual naked instead of permanent remain censored behind undergarments .
Also its is not censoring it is choice. Please keep that in mind. If I draw a woman in underwear or with a fig leaf I am not censoring, I am choosing to present it that way. Censoring would be adding leaves to nude statues of other people.
The Assassin's Creed Valhalla game says that there is 'partial nudity' so that is probably why they have the censorship.
I dont get their appeal of adding false nudity toggle in their game settings while nudist character people are still censored but well,.. up to their strange choice anyways.
Well... Three or more then two years later , looks like my suggestion prediction of this came true ( according to some articles mentioned about in full version going to have full undressed nudity option i heard ), although there's probably no confirmation about if your actually able to be casually nude for in game exploration and combat , i only heard for customization menu feature only. Hope its not one of those odd censorship route where its locked behind for customization and inventory menu only while in game world stays censored (like how it happened to Cyberpunk 2077, while least there's a mod to fix that) .
Edit : Nevermind, its real, nice , glad they didnt follow that type of censorship
