If origins only unlocked as some kind of New Game + mode, then it would make more sense to have unique starts for them.
you get miles more content from having them as companions, playing as them, you essentially just have -1 party member and few (too few) unique dialogue options
Karlach is one big plot hole from start to end and it's best you don't think about it too hard. The moment you start questioning what a infernal engine does to a living being outside of avernus for example. Hint: it runs on souls. So anyone she kills should turn into lemures. But Larian forgot about that.
You can tell she was a late addition, just like Halsien and Minthara.
