Depends. For "casual" users, the in-game mod manager suffices. This is if you only ever use sanctioned mods. Next is Vortex. This is for intermediate users. it gives you a little more flexibility but has issues, especially managing manually installed mods. (This is the one I use.) Finally, BG3 Mod Manager is perhaps the best of them all, but like all high-performance engines of any kind, it requires a certain amount of proficiency to not blow it up. The bar is higher than for Vortex, but the payoff is proportionately larger.

At least that is my take. I'd like to hear from others as well.

No such thing as "sanctioned mods". The mod manager is what's official, not the mods themselves.

Also, Vortex is garbage.

I've been using BG3MM since it was released and it does the job very well.
Still works fine after the official mod support came out, don't really see a need to use a different manager, so I haven't switched.

Easily BG3MM. It was specifically designed for this game unlike Vortex which is hit or miss due to its focus on working with as many games as possible. It has also multiple features built-in like Achievement Enabler or automatic Script Extender updates.

I wouldn't recommend using official in game mod manager either. It is still new, lacking features and the most importantly, has very limited mod support (and the last thing you want is using multiple mod managers at the same time).

The official mod manager built into the game is enough for me for the moment, but if I were to deep-dive into modding, I would go the BG3MM.

The best is manually editing the modsettings.lsx directly with notepad++. it is simple xml.

Otherwise anything else will work. They only automate the editing of the same file. All have quarks, limitation and remove you having control.
