What exactly do the consequences entail? How do you interpret the "scent of the hells"?

It does not seem to be a typical deal where one promises their soul to the Hells. There is no contract. I am simply wondering from an rp perspective.

I'd call it a "dalliance" more than a romance. Mizora will in fact taunt you the next day saying you might want more, but you're never gonna get it, ... cue song ... no risk to your soul. (I also think she's not "really" having sex with you physically, just putting visions of torment and delight in your soul.)

But BG3.wiki says it can jeopardize other romances if you have them. The ones at potential risk: Wyll, Gale, Minthara, Lae'Zel, and POSSIBLY Karlach's. (She can be persuaded to forgive you.)
