There is a special sword you can find and the mind, which grants you a passive bonus if unlocked.
Follow your quest markers or explore more.
Solve the puzzle, it unlocks a room behind it. In that room is a sword that causes madness, and a waking mind.
There's a solution for the puzzle at the Wiki, the one trick to it is you are trying to solve a puzzle in three dimensions, based on a "solution" they show you in only two. (So it's not always clear what nodes are above/below.) Took me a while to figure it out.
Put the waking mind in a nearby interface, make a deal with a disembodied head, and get Githzerai Mind Barrier buff.
Basically, that's the main thing you get from solving the puzzle.
