Do you have the Hammer?

If so, you may have encountered the 'Missing Quest Item' failure that has arrived with patch #16.

The jury is out if it is caused by Mods or is a BG3 code problem.

Yeah I have the hammer. I picked all the usual options and I've never had issues with this before with the current mods I have but I'll try removing them and retrying

Just removing them *may* not be enough. But give it a shot. The problem is just a few days old since Hotfix #16.

Keep un mind that mods which worked before can break with every game update or hotfix until such time when Larian officially supports them.
And a m9d breaking things doesn't necessarily mean the game must crash.
A m8snamed variable or broken symlink in the Inventory database won't be noticed until such time the game looks for something and it's not there because it's flagged incorrectly, due to a mod.
BG3 Mod fixer and the legacy party size extender in particular seem to be causing this issue in conjunction with #16.
As to what's to blame? Official game patch or outdafed (as in last update in August) unsupported mod?
I go with the mod on that.

I like that phrasing, since the jury (U.S.A.) is not to declare whether someone is innocent - only whether they are guilty or not. How it currently looks like in that regards: https://www.toonpool.com/user/589/files/jury_verdict_3362299.jpg
So far there has been only one forums thread with one specific issue to be proven "bug by game", while the rest turned out all mod related.

It seems there are plenty of mod users who never removed mods properly.

Cute cartoon. :)

Roger that no not doing a real 'clean' of a system. It is just not common knowledge (yet) that Larian does not do a full uninstall when removing BG3 from a system. Or Steam does not do it.

There are certainly bugs in the game. But I really am more convinced than ever that most of the serious 'Bugs' are from player choices in the game, or misbehaving Mods.

Oh well. Do what we can. :)

I concur many are mod caused. Or exploit caused.

That said, when people on systems that CAN't use mods (Xbox, PS5) report issues, then I think it's not mods. Then we get into the question of PC users who say they aren't using mods, when in fact they mean they thought they disabled them, and did not disable them properly ....

The thing is, issues can be intermittent. Some folks get crashes in Jannath's House, some don't. It's looking like one key trigger may be using See Invisibility. It's not mods, it's just that sometimes some folks are not doing something (legit) to trigger the crash/issue.

Point being: if everybody is not experiencing a problem, the immediate go-to-next working theory is it's mods. Maybe. Or it could be that the causes are situational. I also get why those bugs are harder for developers to debug.

I will also only point out sometimes mods FIX dev caused issues, too. Notably, Shield Bash not working. This is all on Larian - they SAY so. However, there is now a mod to FIX the problem at Nexus. Use it or wait for them to do it.

In short, software is complex, and the causes for problems are mutlicausal.
