Approval/Disapproval in camp or in the party.
Do actions cause approval/disapproval change for npcs when they're in camp or only when they're in the party actively alongside the player-character?

Your actions in game affect all companions even if they aren't with you. I won't spoil anything, but they will leave your group if you do something they hate, or be hostile towards you if you hadn't recruited them yet. I ran into this multiple times playing Dark Urge. I only have four playable companions right now.

Spoilers don't bother me but I have always been annoyed at games where characters didn't react to something later just because they weren't present for it. I mean I always figured details would be discussed later on or maybe characters who weren't present would hear about something that happened previously. And then a reaction would take place.

Like Wrex in ME3. He didn't actually witness Shepard either curing or sabotaging the cure to the genophage but he did find out if it was sabotaged and he damn sure reacted.

The big plot choices do affect all companions, but if we're talking about casual Approval/Disapproval, then those in the camp don't react, afaik.
This is glitchy though - the ones that you have just dismissed may still react, but if you save and load or go to a new area with a loading screen, then the game properly registers that they are no longer in the party, so they don't react.

One example then. I chose to play evil Dark urge, and sided with Minthara in Act 1. So I wiped out the Grove. Wyll left my camp even though he wasn't with me, and when I went to recruit karlach after the battle she screams about what I did, and she attacked me because of that choice. The companions who where with me did react to my choice to wipe out the grove, but basically said I'm the lesser of two evils, and made it clear my character is only useful to them to achieve their own goals. Those characters also have options to be evil as well. I thought it was really amazing how there were consequences to my actions like Karlach attacking me.

I'm now in Act 3, and only have four companions left based on all my in game choices.

The way I get around a lot of the disapproval is I use the character who would disapprove of an action to actually perform the action thus eliminating the penalty as they don’t disapprove of themselves. It works on a lot of different actions.

That sounds like a horrible oversight on the part of Larian. To allow the player to get an npc to carry out an action which is against that npc's stance doesn't make sense. Or rather it makes sense when the origin character is the one being focused on but not when the origin characters are members of the party.
