Wait a minute... [act 3 spoilers]
So we all know Orin kidnaps one of your companions and threatens to kill them if you enter her temple before killing Gortash. Now let's say -hypothetically- you go after Orin first and she kills the hostage.

Couldn't Withers just... Y'know... Uno reverse card that whole situation?

You can thunderwave Orin into the chasm instead of talking to it. The altar key and the Netherstone dagger will remain on the ground to pick up.

I'm not surprised there's numerous options. But if the hostage WERE to die, can Withers bring em back?

Haven't tried, but the game might slightly break (the alive companion will remain a follower) if you finish the battle outside the temple, e.g. by kiting.

No idea, I've never been terrible enough at the game to get the companion killed.

I don't think so. It's a plot death. It's in sort of the same category to why you can't use a scroll to revive an npc. Games in general with revive mechanics get kinda weird when it comes to a plot having stakes. Maybe Orin's altar is special and makes them stay dead?

For your answer likely they stay dead, seeing I messed up saving Asterion on my first attempt and he died permanently. So I assume same idea if she kills your companion.
