Karlach romance/ Shadowheart Flower bug?

It's not as if I was achievement hunting but if I love a game the way I love BG3, I like to make a strong effort to do as much as I can without running out of fun and so far I can still go for another round.
There are 2 achievements that grind my gear because I should theoretically already have gotten them.

1. Karlachs romance
During my very first playthrough I fully intended to romance Karlach because I like her character so much. She's on fully good graces with me (all the way to the right) but I still never had the option to romance her until the very end. Is there anything I'm totally overlooking?

2. The Flower problem
This one is even more annoying. During my last run (Tactician) I romanced Shadowheart and when I get to the cave with her favorite flowers, she never wants them from me. I can't remember if I ever had her tell me that she loves them but I have NO CLUE why. I basically always exhausted her dialogue option?!

I also was not able to gift her a flower. I wonder if maybe my regular and frequent use of the Disguise spell may have glitched that out and caused her to not realize we'd had that conversation before.
