I assume Karlach is a Barb still? Did you revise her ability scores to proper distributions to something like 16 STR, 14 DEX, 16 CON?
Assuming Barbarian Karlach, I find it hard to believe she apparently misses so much with Reckless Attack.
If survivability is an issue, try Bear Heart Barbarian. Half damage from everything besides Psychic.
Early Game is rough learning the ropes of how to make your gang more effecient but Everyone of them has a moment to shine.
and, as far as missing goes, i have that same problem with SHadowheart's stormtrooper caster accuracy until I get to act 2 (seriously, she hits more often with a light crossbow then Guiding Light (I think that is the spell? the nuke that gives advantage on the next attack roll against that mob?))
But if you do not like her as she is, make her a fighter. I use her as a Berserk Barbarian, but I am not sure if I like this subclass.
She also makes a very darn good paladin
gets me thinking about a barbarian/druid hybrid
wonder how her soul coin ability would work combined with owlbear form?
(raises another question, can rage be used in wildshape form?)

Once you give her ring of flinging, gloves of uninhibited kushigo, returning pike, some +1 spears, potions of speed, tavern brawler, hill giant elixirs and stack her up on at least 3 crates, she will be going crazy in act one, doing like 60 damage per turn with more range than a bow
