"Friends" and "higher difficulty"

Hello everyone,

i was wondering the Cantrip "Friends" states "In higher difficulty modes, the target might accuse you of enchanting them."
what counts as "higher difficulty" here?

I can't say for sure, but I'm playing on normal and I didn't see any punishment for using the spell on NPCs.

However I think I did see a disapproval when I did it on a party member, so I just savescum a success without using the spell.
