Impress the Murder Tribunal password - bugged? (warning spoiler)

I did the "Investigate the murder" quest and found/saved all 3 alive victims on the list of targets. I killed Dolor and saved Figaro, then I searched Figaro's body and found a note and a bag containing 4 severed hands. Then I got a cut scene where the investigator lady appeared and told me to find the Murder Tribunal. So I went to the little shack and found the button behind the painting, pressed the button, and the book shelf moved to reveal a hidden door. But when I select on the door and passed the checks, it tells me I need a pass code. I looked up some video guide and written guide for the quest, and it looks like you are supposed to get the pass code when you kill Dolor, which I have already done so. But strangely I still don't have the pass code. Also the journal for "investigate the murder" didn't update, instead I have two entries; one entry says I must find Fiagro, the other says I stopped the killing spree by taking down Dolor and I must find the Bhaalist cult. But I have done both already and still no password to the hidden door to the murder tribunal.

Has anyone encountered this issue? Perhaps the quest is bugged?

I have this same bug, I have no Dolor body after killing him without him teleporting.
