Edit: there's also the int circlet, boosting it to 18 and a pair of gloves that boost dex to 18. Might be other pieces of gear, but I didn't find any others in my playthrough.
If you pass a DC 25 religion check you can pray and the increase instead becomes +2. This can be done per character.

The Ghoul horde in Act 3 is by far the best permanent buff you can get. It made holding onto that book for 90 hours worth it.
Are there really no negatives for Astarion biting that person for the potion? He mentions her nasty blood so I figured he ends up with a debuff from it.
