havens fairly often to me, end up blowing through things like flourish really quickly, finishing off enemies.
Happens a lot to me too and I have a +5 on my charisma as sorc lol
The Die have been making me so paranoid. I swear sometimes, the game just rigs the die I don't trust them at all. Rolling a 1 is like a 1 in 20 chance, I rolled it (1) 11 times on the same lock WITH karma die on. Like wtf?
Used Roll20 die for years, never had this issue with rolls.
Paranoid theory; They know dice rolling is addictive, artificially mess with the die to keep people playing longer and longer as it looks good on metrics.
No and that knocks them out also.
the good news is that someone else out there just crit 11 times on one of their characters because of your sacrifice.

The Barbarian has such a feature but I wouldn't think 80% of the enemies encountered to have such an ability.
I'm not having this issue on my singleplayer run, it just seems to be in multiplayer on Tactician.
You could try to deactivate Karmic Dice in the options.I had the same feeling and after i switched it off it felt a lot better.

I've often missed my attacks with 80% chance to hit. I do feel like I'm going insane but I just had a goblin take about 12 attacks from my party with an average of 65% to hit and they all missed, yet every attack against my 21AC hit without issue.
Concentration rolls also seem scuffed. Enemies seem to always succeed where I and my friends will always fail even with advantage.
We always look. Though I feel the examine on enemies is meta. Unless the information is being left out (which is the case with some feature/spell descriptions) there is nothing that I've encountered with a feature/buff/racial that would allow it to drop to 1hp rather than die.
Regardless the damage die would show the damage going beyond 0hp but them bouncing back up to 1hp.
