IIRC dead merchants drop their most expensive items.

It's not random what loot drops from traders. There is some kind of priority. It you haven't traded with the trader, they will always drop the exact same loot. If you have traded with them, sometimes the loot the normally drop will be replaced by something you traded to them.

Anyway, that item has always dropped from that trader for me.

Not sure if this was changed in patch 8 but you can sell any bag/container to a trader, put all their items inside it, even their gold. When you kill them, they'll drop the bag with all the items inside.

I got it for my monk and I purchased it from her (100% sure of that, I never killed her & saw her again in Act 3). The thing is, I don't remember if it is before I gave her the (fake) githyanki egg or after. I think the store inventory changes for better items after you give her an egg (the fake one or the real one, doesn't really matter, she doesn't know if you pass a check).
