As for where you use them, find the altar where you can insert a gem.
-Use the first to activate the pedestal near the giant statue of Shar.
-Seek the fourth from any missed trials of Shar or speaking/killing the Orhanc & taking his guarded Umbral Gem by his corpse bed.(Killing the rats->final Dark Justiciar will help convince the Orthanc, but Raphael will still claim he. Though perhaps he could be useful later..)
-Ride the pedestal & use the final three to open the door the point of no return.

This gauntlet is really pissing me off. Okay I finished the faith leap trial, the self same trial, and the trial where you have to sneak through

Orthanc is a location in The Lord of the Rings. ^^
And yeah, that guy has the fourth gem. Or rather, the gem is on the ground near his bed. Just for fun, you can cast Speak with Dead on the bed, which is made of corpses.
In all seriousness, the large, boasting of wearing a King's skull as a codpiece burly bio & his chemically induced Displacer Beast lover is obvious in presence &...well lawful/neutral evil both
Else Duder(OP) will soon finish the vexatious gauntlet & finally start Act 3-& there will be much rejoicing.
