Wyll + Ansur Quest (there will be SPOILERS)
I killed Wyll inadvertantly in the early part of the game. And I forget how to get the Ansur quest (I did have Wyll in a previous runthru). So, I don't know if you need info from Wyll to get to the Ansur Quest area below the prison. Any insight appreciated!

IIRC, you get the Ansur quest from his father, Duke Ravengard, that is, if you manage to save him from Gortash's underwater prison.

Technically you don't need to be quested to go to the Ansur Quest area but if you are looking for the book Legend of Ansur you can get it from Wyll's father or some members of the Flaming Fists in the city. I think I got mine from the Flaming Fist.

Spoilers?

I usually get offered the book Legend of Ansur twice. First time when I run into some Flaming Fist in the city who accuse me of killing Duke Ravengard (and when I ask them who told them I did any such thing, they tell me it was Mizora - oh cool, so you're taking crime tips from devils now) (usually shortly after Wyll rejects his pact) ...

Second, when I rescue Duke Ravengard from prison ... the game doesn't seem to check that I already have the Legend of Ansur, and he once again wants to talk to me and Wyll about it and offers the book a second time (though I don't get a second copy of the book).

I suspect - don't know - as said above - if you rescue the Duke from the Iron Throne, even if Wyll is dead or gone, he will offer you the book, starting the quest. But, of course, I also suspect if you already know how to get into the Wyrmway, the quest will "auto start" simply by going there, as suggested above.

Devils don't lie, though. Bending the truth or withholding aspects of the information is a different matter (not debating the philosophy of how those are still lies) so being told you killed Ravenguard from a Devil has a lot of weight to it.

"Lady Mizora" is in disguise when she spreads her little rumor-bomb. They don't know it is a devil who tells them that.

Spoilers again.

Mizora certainly leads you to believe Gortash has had Wyll's dad killed if you reject the pact. And in fact, you pretty much have no reason to believe otherwise, up until you to go to the Iron Throne and find he's there alive. So, yes, I suppose the FF could be believed that you did something that led to his death, if you also didn't know otherwise. However, ... I think even in the convoluted legal code of the city, refusing a devil's pact leading indirectly to another person's death is hardly murder, LOL.

Obviously, she has to present it this way to Wyll -- either stay in the pact, or your dad is dead -- although I suspect she knows he's still alive, as a devil she can scry distant locations, etc. Maybe we could put this in the "withholding info" or "bending the truth" category.

Anyway, I imagine rescuing the Duke with Wyll dead or gone is actually easier, as there will be no interference in the rescue by Mizora.
I've been wondering if it is possible to side with Karlach over Wyll, rather than just convincing Wyll to not kill Karlach. I've never tried it.

I suspect the Duke will give you a book whatever circumstances. I received a second copy of the book after finishing the quest. The Duke, when he gave me the book, was not aware I already killed Ansur.

So THAT is definitely a bug, although I think him giving you the book even when you already have a copy is one, also. (Although, again, I guess, if he doesn't KNOW you killed Ansur, him telling you about him is not a mistake. But I suspect you can't even tell him you already killed Ansur, even if you wanted to.)

Not a big one because dialogues with the Duke were back on track not far later. It's like the game have to catch up.

So yes you can technically, but you're on your own, it's not Karlach' demand.

Would you happen to remember which Fist talked to you about the book? Or at least which area of the city she/he was in?

I killed Mizora in the pod, not realizing it would IMMEDIATELY summon Wyll to Avernus. I thought it would be a troup of powerfull hellions to magic in and capture him.
