So I just did a run with astarion where I hid the fact astarion is a *spoiler* until act 3 SPOILER
Even at act 2 I had the prompt to "tell the others you are a vampire" and so, usually kept off-screen/camp sending companions elsewhere to make sure they don't see a single bite.

Soo what is the result? Well for anyone wondering:
Nothing really happens, in act 3 for me the prompt simply disappeared, maybe at gurs there would have been interactions, but I am unsure as I went straight for cazador's palace, but there wasn't anything big like "woah, are you a vampire?" or anything of note. I am unsure if maybe I messed up somewhere, but I am certain no one saw a single bite and so. Afaik even when the drow requested vampire bite and so I sent my companions away and after that I could still tell them. If anyone had any different experience who may have ever tried the same please tell me, that was my experience.

Wow, good work. I wonder if they just didn't expect anyone to get that far hiding it lol

I also don't have tadpoles and have the mind shielding ring too, so I will upload on that 2, you know 2 birds one stone haha. (fun fact, this is also a honour mode run which is why I cannot really check back on the gur stuff lol)

Honour Mode too is straight up masochistic lol. You must be very meticulous about it.

The vampire that never was. There should be an achievement for 100% avoiding origin plots; that takes effort.

While I do agree it would have been fun if they had taken this possibility into account, if you play as Tav/Durge and recruit Lae'zel before Astarion, she will compare his appearance to that of a wight. Obviously she's on to him from the start.

If you bite a companion as Origin Astarion, I believe the following morning round of conversations where you admit you're a vampire has Shadowheart responding with something along the lines of "well, duh, we had figured that out even before you tried to bite one of us."

So two or more of the companions do suspect Astarion isn't "normal". Dhampir are a thing in Faerun so at the very least they may have assumed him to be a daywalker.

It's almost as inconceivable as two people meeting in a public multiplayer session and actually wanting to communicate with each other!
