Permament removal of a party member that I've found in the EA are the following:
Handing Astarion over to the monster hunter.
Peering into Gale's mind and having him catch you doing it. He leaves if you can't persuade him to stay.
If you long rest after Lae'Zel runs to the Gith patrol you,ll find her dead the next day.
If you've used the tadpole enough to start suffering from nausea early on, Lae'zel will offer to kill you. You can convince her to give you the knife so you can kill her instead.
Stake Astarion when he tries to feed on you during a long rest. Failing to kill him with the stake will still cause him to flee.
Wyll abandons you and fights to protect the Druid Grove if you side with the Cult of the Absolute.
Not saving Shadowheart in the pod and not recruiting her at the grove and then long resting will have her arrive at your camp going feral and will try to kill you if you can't snap her out of it.
Only did that once and it was Astrarion. His dead body was in my camp for the rest of that play run. :P. But that may not be the case for the final version.
You talk to him. He wants 200 gold for each one you want to rez. And yes, they will appear back in your camp.

However, I wouldn't do it until you see the dialogue option for it.
Don't go around killing everyone before you have a good reason to. You'll miss out on a lot of storytelling. I think the first companion that I had the option to kill was Astarion, because he has a serious addiction problem.
If you kill them, bedtime stories are over.
I'm sure Larian has fail-safes throughout the game should you be susceptible to random acts of violence.
Kill her when the next time you meet her after getting off the ship. Loot her body for her armor, give it to Shadowheart as a nice AC upgrade. It's not going to keep you from finishing the main story for your char.
They even confirmed the lewdest act of all with Lae'zel: Hand-holding.
That said: definitely do this in another playthrough.
I loved killing pretty much everything in my path in my second or third playthrough.
