Yeah, though I doubt he would risk so many resources to deal with epic PCs.
In the novel Elminster in Hell the Simbul kills a fair share of high level devils who are not so easy to replace. Especially if they die in Hell where they won't come back to life unlike in the material plane.
Pit fiends are not exactly expendable. Especially the more loyal ones.
To be clear, I think he can do it with less resources expended than that. But it’s an option he has, and if a player wants to “take a swipe at him” and they don’t have a god willing to go to war in order satisfy their desire to take a swipe at The Supreme Master of the Nine Hells, they’re facing an arbitrarily high number of arbitrarily high CR encounters well before the man himself gets involved. GLHF!
With a lot of prep and a LOTTA CHEESE, you can.
The rule if, if it has Hit Point,s it can be killed.
But be prepared for a TPK or at least loosing half the party.
Tiamat in her True form, is a 5 headed dragon, and High level monsters get "Legendary actions", wich are special actions that allows them to act Off turn.
A monster with Legendary actions, can take an action at the end of Another's creature Turn, the # of Legendary actions are limited and you can make the same action, only once per turn( turns being each characters turn, when each character's took their turn, then a round has been made, its an important distinction)
Usually these kinda monster has 3 Legendary action points, with a list of 2-3 actions that they can take, where the simplest ones cost 1 Legendary point and the other 2.
Since Tiamat has 5 heads, she has 5 Legendary points and using one of her Heads to spew a Breath attack, costs 2...
She also has a mild Magical immunity since only spells of 6th lvl or higher can actually affect her, anything else is ignored.
A lot of Immunities and resistances to Non-magical damages and conditions and a whallop of HP (around 600hp and 25 AC...
Also One reaction per Head and per creature's turn...
Regeneration (30hp per turn), 5 Legendary resistances (Legend resistances, allows a creature to automaticly be succesfull on a saving Throw)
3 Attacks a turn, a Frightfull Presence Aura that can inflict fear AND her breath attacks do between 16D8 and 26D6 dmg...
Oh and 3 times a day she can innatly cast Divine Word and most of her spell DC's are at 27
In her True form, she is a tough cookie to crack, unless the party has some artifacts, some powerfull spellcasting and powerfull allies...
Her Avatar Form, while being also tough is a bit less Dangerous, by a small margin lets say
Stilla round 600hp and AC25, and if she reaches 0 HP instead of dying her HP jumps back to 500, and she can use 2 more abilities, wich are called Mythic Actions.
This kinda mechanic is akin to Game boses that has a Second phase.
She doesn't have the Legendary actions to spew her Breath weapon anymore, but her Breath weapon "Chromatic flames" is still a 300 foot cone that deals 11d12 of elemental dmg of the Dm's choice with a DEX save of DC27...
Her Mythic actions are "Hurling through Avernus" where she sends a creature through the hells, dealling 8D10 Psychic dmg and the creature is banned into Avernus till the next turn.
And Chromatic flare, wich is a lesser version of her Chromatic flames, only dealing 6D12 dmg...
Tiamat doesn't have such mechanic
But the Purple Worms and the Tarrasque does.
I mean, the first entry into the Baldur's Gate franchise in over twenty years, should've been an "epic epic scope" they really should've done twenty in the base game. This game gonna be played for years most likely, give people the full experience in the base game. Hopefully they include a campaign editor, not that it will really allow people to make a legitimate level 20 experience.
Depends on your DM.
But since usually this type of thing is covered by the statblock of the creature, so if there is no such thing, then its not possible.
Not that its is impossible from a "logical"' point, but your DM can rule it out to keep things simple and smooth.
LIke "Yeah you can try that, but Tiamat being a Queen despite all appearances, has learned to chew her food and not just gulp it down Whole, still wanna try?"
Purpls worms, Illitid Worms and Tarrasques HAS mechanics like this build into it, cause they are creatures that is expected to swallow creatures whole.
Dragon despite their size, not so much.
Usually in the Tarrasque and Purple worm cases, when they deal a special attack and hit, the Target as to fail a save, usually Strenght/Dex or Con, and it gets swallowed.
While in there the creature as total cover from any attacks and effects going on outside and he can't aim anything from the inside.
The creature is blinded grappled/restrained and is inflicted with XX of Acid damage at the start of the turn.
If an attack from the interior deals XX damage in one turn, then the Tarrasque/Worm, must take a Constitution save, if it fails it regurgitate its stomach contens out.
With the character reappearing in a space next to the Tarrasque/Worm.
With my Party in our campaign, we encoutered an Illithid Worm in a Githzerai Ziggurat floating in the Astral Sea( we're at the stage where we are Spelljamming).
Worm swallowed our Gnome Artificer(first encounter the player participated in the campaign, brand new player) took 40 acid dmg a turn was almost out of HP, when my Dark Knight character ( paladin/Hexblade) went and Critted on a Divine Smite, obliterating the Worm...and the Gnome (for Fluff reasons it was due to the Implosion from my Smite that the gnome got unconcious, but it was due to fall damage).
