I haven't played in a long time. Should I continue my previous save, or start a new game?

I haven't played in significantly over a year, in large part because the game simply got too laggy in Act 3. Now that I have a new PC that should be able to run the game, I'm thinking of coming back to the game, but I'm wanting to know, has the game been changed enough in the past year+ that I ought to start it over from the beginning, or am I fine continuing where I left off in the middle of Act 3?

The game hasn't changed hardly at all, so if you make all the same choices in Act 1 and Act 2, then you would end up back where you were already.

Ah, okej, good to know. I remember seeing a lot of patch notes over the months and thought some pretty significant stuff was happening based on their intimidating length.

If I were in your shoes I'd either finish the current run or wait for patch 8 and then start again.

Most of the actual content changes have been to the epilogues of the game, and you haven't gotten that far yet. Honor Mode and Custom Mode got released. Kissing animations were improved. Minthara can now be recruited to your party without raiding the Grove, if you knock her out instead of killing her. The rest have mostly been bugfixes,

I faced the exact same issue a while back....

In the end, I decided to start over with a new play through, just because I found that I was kind of fuzzy on the choices I had made, and what I was doing exactly at the time I originally left.

It was just easier to start over, than try to remember everything I had done and why.....
And, I don't regret that restart at all.

wait for patch 8 then go all in with a new save

You could finish the act 3 save (if you still enjoy it) and then wait for the new patch and start a fresh save once it comes out.
