Frankly, I find the outlook stupid 'cause not everyone gets a happy ending, just one that's easier to live with.
Shadowheart loses her parents and has to contend with her memories gone forever while relearning herself, getting in touch with her original faith.
Lae'zel has to give up everything she learned in her life and her faith to rebel against her own people she prides herself in.
Astarion may be free of Cazador but he is still stuck to the shadows after getting to experience the sun for the first time in 200 years
Wyll is arguably a potential good ending outside of being a devil for the remainder of his life.
Gale, depending on what you do with Karlach is either dead or alive teaching.
Everyone gets a better than bad but not entirely good ending but people don't recognize how everyone is screwed over in their good endings besides Karlach so they don't pick Karlach. Karlach's is the easiest to pinpoint because she directly says she refuses to go back to hell but we make her to keep her alive.
I can't get achievements (they're disabled but I don't care), I knew she's doomed (though less doomed now that there's an Avernus Epilogue), and I've romanced her. More than once. Once with a male Tav, once with a female Tav.
MHO: now that there's an Epilogue, her going to Avernus is a less bleak ending than becoming a mindflayer. It also holds out better prospects for romancing her. The Epilogue hints her and Tav may well be able someday to leave Avernus, and maybe even move back to the Prime Material, since she may indeed be able to fix her infernal engine.
You don't have to break off your romance with flayer-karlach ... but ... even she admits ... it's gonna be strange going. Same thing as with Emp ... mindflayers don't really appear to have human emotions or feelings anymore, and don't have, uh, genitalia.
BTW also MHO -- there is no Emperor "romance". There's a seduction scene he uses to manipulate you. It really shouldn't be called "romance" because it's not. I suspect it's a popular choice for some players because a) they wanna see what will happen with squidward (the answer is some hint he's doing some tentacle hentai to you, then fade to black) and b) they want the achievement. As said.
I also don't think anything in the physical world is taking place anyway, it's all just an illusion in your mind. Like the kind of psychic sex "David" used to have with his girlfriend "Syd" in the FX series "Legion".

Especially in Act 3, the fact that she's confronting her mortality more and more imminently starts to change a lot of the "happy go lucky" attitude of earlier acts. She knows you don't have a future, but she sure wants to envision one with you, anyway.
... as I said ... spoilers... the Epilogue holds out that possibility. (I don't really view spending the rest of your lives in Avernus as a "future".)

While I don't think this is important, I am a woman playing a male character.
Everything she says and does is the opposite of how she was seen acting during her active time as a slave mercenary.
"Ohh, how much I dislike my previous boss, ohh noooes, how bad they were! Thaaaaaaanks for rescuing me. I am suuuuuuch a good person, look at me being soooo reasonable and calm, nice to everything and caring about everyone. I could neeeever touch anyone else".
Yeah... the only reason she is even some option is due her model being halfway decent in comparison to all the other disfigured creatures out there in the world.
What are you talking about, we don't see how she acts during her slave years. Those who did see her during her slave years seem to attest to her having being good natured back then too.
I just don't buy her whole, "Hey guys, i'm just your friendly neighborhood demon from hell, like literally, hehe, but for real, for real, yeah i'm a serial murderer straight out of hell, BUT! That's all behind me now, and uh, I'm just trying to get this softball league i'm working on off the ground, so can i sign you guys up?"
Like hun, even if you're being genuine, you kind of need to embrace what you are and not come across to everyone as a freaking church deacon cat lady.
So the problem is you guys are projecting whatever the hell you wish into gaps. At what point does she tell us she is a "Serial Murderer"? She wasn't out murdering people. She was in a war. Against the Abyss. Demons. It isn't like she was tasked to go kill innocent people, unlike Wyll (whom, btw, Mizora all but confirms Karlach is an innocent by never correcting Wyll on her deeds, only that she is heartless by virtue of having no physical heart. If Karlach was as bad as you folks try to paint her Mizora wouldn't have needed to use a loophole in terms to peg Karlach as "heartless" to fit the nature of Wyll's contract). She hasn't fought people that were not related to this particular war, therefore, she wouldn't fit the description of a "Serial Murderer".
And just because she is as soft as a kitten, doesn't mean she cannot be hard as nails as a soldier. Two opposing things can be true at once within a person. It's called a dichotomy. Look it up.
Also, what do you mean, "Neighborhood demon from hell", she isn't a demon??? What???
Put it in better terms, developers tried so hard to make her inoffensive and likable that they actually achieved the opposite result.
Likeable might be subjective (some people might like Karlach, occasionally for reasons that are meta) but the OP offers adequate proof that only a minority has the corresponding achievement
To finish her romance requires Dammon to survive Last Light Inn, and for people to have fully upgraded her engine. Dammon dies at any point, her romance stops cold turkey.
And using the achievement as a benchmark when no other companion has a completed romance achievement but her is bias statistic.
Per Larian's own statistic, Karlach is the next most romanced character after Shadowheart, then its Lae'zel. If 5% of people getting Karlach's achievement equates to 17% of the playerbase, than most people do not romance the characters in general.
