Is it okay to not do my companions quest in act 3 ?
Basically, i'm playing with wyll, Karlach and Gale, is it ok if i choose to ignore Astarion and shadowheart quests in act 3 and keep those for my second playthrough ?

Yes, depending on what you mean by "OK".

Doing the quests might give you rewards (dropped loot) and give you extra allies you can summon in the final battle. But the game is completely winnable without doing those quests.

You will just miss on one of the best shield and some permanent buffs if you don't do Shadowheart's quest. Unless something obvious, Astarion quest is a little bit a waste of time.

Kazador also has a neat dagger for ranged attackers/casters as a loot.
You can play your game however you want.

I don't do all companion quests each playthrough. First was Lae'zel, Gale, Karlach, then next playthrough I did Evil Shadowheart, Astarion, Minthara, then I did Wyll, Jeheira, Halsin, +Minsc (at the end I just swapped him out).

Kept each playthrough fun.

Next evil run I think I'll play as Gale or make Gale evil.

Nothing is forcing you to do their quests. There is more than enough Exp to go around in Act 3. I never use the allies in the final fight, so you don't lose out on much for not recruiting them. The gear... yeah the best in shield in the game is nice and all. But not necessary to beat the game.

Occasionally, their personal quests tie into their romance if you are romancing them, and you may need to do them ... if you want the 'optimal' romance ending.

I always do them for RP reasons, and because I often enjoy the fights that ensue (the fight in the Temple of Shar is one of my favorites in the game), but no, you don't "have" to.

Not doing them may affect how their endings and epilogue work out ... but again, that's not a "must" thing, just noting this since you asked. If you do want certain endings for them, you may have to do their personal quests.

And then sometimes the ultimate resolution depends on choices you make after.

For example: so if you complete the Wyrmway/Ansur, for example, Wyll can choose either to become the Blade of Avernus, or the next Grand Duke of BGC. You can let him choose himself, or "influence" his choice. I find if you leave the choice up to him, he usually chooses to become the Blade of Avernus.

If he becomes the next Grand Duke, well, congrats, you now know the new leader of the city.
If he becomes the Blade of Avernus, he will hunt down Mizora and other creatures in the hells. He no longer has his warlock powers, so he's switched to being a ranger.

Everything in the game is "okay".
It's ok for a 2nd playthrough when you *know everything*, it's ok if you don't care that much or if you just role playing quite a chaotic player. Only thing you lose is some companion lore, note that some companion's side quests can't be ignored whenever they are part of the main plot.

You don't even have to take them as companions at all in the first place.
