2) The weapon text says "can". The wiki says "potentially". I have had it go Mad and not go Mad after loosing the blessing.
3) It is very easy to avoid, that is keep Yeenoghu's Blessing.
How do you know it does not do damage on first turn after going mad.?
In combination with the amulet of healing, it gives you 6HP on every hit. I also bought the ring from Larokkan tower, which gives 1-4HP each round, so I was unkillable.
Same way Rage knows you've tried attacking or not if you apply Rage AFTER attacking that turn. Status checks your actions prior to applying its effect.
Like any bludgeoning weapons. Any flails, warhammers, mauls, clubs, light hammers. maces and greatclubs work well against Grym. Some are better than the others because they got inherently better base damage.
Punch Drunk bat never left Karlach's hands one run. She wasn't the hardest hitter, but she was the most consistent, a crowd controller, and still took out more lives because of being both consistent and crowd control.
This was on Tactician. Ain't tried it with Honour rules yet as I build her completely different on Honour.

Eh? How is she not? She is the most damaging Barbarian, hard stop, and gains a resistance to the most prolific damage type in the game without needing to go Wild Heart.
