Cull the Weak bugged?
I have had this power since the moment it became available and in 20 hours of playtime it has only triggered once.

It's activated, and I regularly notice that I have enemies down to one hit point but it doesn't trigger. I have spent 5 parasites, so doesn't that mean anything with 1-5 hit points should instantly die?

Could I be doing something wrong or is it bugged? not sure how I could be doing something wrong, as it is passive.

Make sure it is turned on.

It automatically turns itself off if your party switches to non-lethal combat (for obvious reasons), but it doesn't automatically turn back on again.

There are certain kinds of attacks and damage that don't seem to trigger it. So it's not 100% reliable, but it works for me most of the time.

It's always on, I keep checking. My party has never switched to non-lethal.

Seems to me 99.9% of attacks don't trigger it. I read that it was bugged a while ago bug that it was patched. I only got the gaame the other day so I should have the patch too.

Cull the Weak only triggers on main damage dropping a target below the threshold, not secondary damage. If you stab someone with Sneak Attack and the main weapon damage drops an enemy to 15 HP but then Sneak Attack's damage drops them to 5, Cull the Weak won't trigger.

If you are fighting undead they will always survive at 1 hp due to their "undead" ability/nature.
It's not terribly useful when you have 5 abilities activated, but by the end of the game it routinely kills enemies.

Is it normal to have had it for 20 hours and only activate it once? Judging be the description alone, I should have seen it work more than once. It's been incredibly common for me to have several enemies in a fight below the threshold and yet it still doesn't trigger.

Good to know, but I don't think it's that either.
If you toggled non lethal attacks on at some point. It disables cull the weak. So when you go to turn off non lethal you have to toggle cull the weak back on for it to trigger.
