Ethel's HEALTH is tied to the mushrooms. And that's only in act 3. Media. Literacy. Yall. She's just enchanted them to help her not fall to 0 while in the PMP. That being said, there is no official lore that states when all fey die they go back to the Feywild like how fiends go back to the hells. On top of that Larian worked with WOTC who provided lore writers to confer their setting and story. Larian wasn't in a vacuum, they were very clear that they had a team from WOTC helping them stay true to the lore.
