You cannot replace each

You cannot remove Volo's eye. You can get auntie's eye on a different character, though idk why you would want it tbh.

The strange thing about Aunty is being Fey, she should reincarnate in the Feywild if killed on the Prime Material, yet the game goes to great lengths to tie her immortality into a bunch of mushrooms ... ?

Yep, sadly your knowledge is superior to the writers of BG3. There are a bunch of things like that.

Or - and I'm just guessing here - Ethel is just lying about the mushrooms, like she lied about everything else?

Ethel's HEALTH is tied to the mushrooms. And that's only in act 3. Media. Literacy. Yall. She's just enchanted them to help her not fall to 0 while in the PMP. That being said, there is no official lore that states when all fey die they go back to the Feywild like how fiends go back to the hells. On top of that Larian worked with WOTC who provided lore writers to confer their setting and story. Larian wasn't in a vacuum, they were very clear that they had a team from WOTC helping them stay true to the lore.
