The journal isn't helping with the note, but what it means is that you don't have any way to kill the general yet.

Yeah, that's what has confused you, the Act 2 narrative assumes you will talk to her and she will send you in search of Balthazar.
I think if you thoroughly explore Balthazar's rooms in Moonrise Towers you will probably get an alternative trigger to go find Balthazar and the Temple of Shah. That is your route to Thorn himself, but you want to complete anything you want to do at Moonrise first.
The game in general is very open in terms of quest design, 99% of the quests are optional and can be skipped / ignored and the main story objectives (killing 4 bosses) can be done without any quests.

I doubled back and took the mountain path after going most of the way in the underdark/grymforge. I sided with the Harpers at the ambush and repelled the attack on the inn, ive freed both the gnomes and the thieflings, both of which are safe at last light, i checked in with the gnome who gave me gold despite being ungrateful, but he still has a quest marker on the map, the theiflings asked that i find the guy who went looking for them but he has no map marker. Ive not yet encountered the Nightsong and I dont even really know what that is. Right now, the journal entry just states that ive killed z'rell and thus cannot accept the quest from her and "need to find another way"
He's out on the main Shadowlands map, you probably haven't explored that bit yet.
As I said above that's what happens if you kill Z'rel before the game assumes you will.
I did say above that it's probably possible to activate the main quest line again by exploring Belthazar's rooms in Moonrise Towers (first floor, which Americans call second floor for some unaccountable reason). Don't worry if it doesn't, it'll re-activate later anyway.
Anyway to reach Nightsong, which is basically the route to Act 2 finale, you will need to find Rafael who is hanging out on the main map in a spot you obviously haven't explored yet.
Your game is still in fine state despite you not getting the usual quest from Z'rel. Your best bet at this point I think is to make sure you explore the whole Shadowlands map and also every inch of Moonrise Towers you can get to (with the exception of the roof which is were Thorn is at this stage and as your journal will tell you he is currently unkillable).
By doing that you will automatically find both the tieflings friend and Rafael and also get to clean up anything else you haven't finished or haven't found yet which would be wise to do before you delve seriously into the Nightsong business.
