hmmm.. wouldn't that get very clunky in some areas, blocking your view and preventing you from moving around without picking up / interacting ?? D:
I suppose they could have a toggle option that it's always ON and then you press Alt to switch Off.
But other than an accessibility issue, due to inability to use mouse and key at the same time, it might not work as you wanted.
You can always take a look at mods to see if any have that option. Failing that, i believe there is a way you could try to have a coonstant Alt button press by using the Accessibility options in Windows for Sticky keys (assuming you're using windows).
Failing all that, a simple heavy pebble on the alt key works like a charm i guess
No to both. What you can miss is optional and not everything is garbage.
Cursed are those with OCD once they figured it out that not everything is highlighted with ALT which is of value.

The ALT key isn't really required at all, if you hover your mouse cursor over a wide area, interactive items will show up, once the cursor zooms over them. Yea, pressing that key helps, I get it, but still not really needed for people, who "search their surroundings frequently and thoroughly". I guess not many people do that, because yeeeey, the ALT key exists.
