Shadowheart romance don't trigger
Beating a dead horse surely, SH never had any romance option for me
I'm in Act3, she's got her white hair, and 100 affinity
But I'm in a relation with Karlach (previously with LaeZel but it was just an affair lol), does that cut off any romance with SH ? Will it trigger if I break with poor Karly ? ^^

Iirc continuing most companions romances in act 2 locks you out of others. The only poly companions are Shadowheart, Astarion, and Halsin. From my experience if you were romancing multiple people in act 1 then act 2 romance progression will reach a point where for Karlach, Lae'zel, Gale, or Wyll they'd ask you to either commit to them or break up. Based on how you're describing it though, is the trouble that you waited to try romancing Shadowheart until act 3? It is likely that Romances for companions available since act 1 require the romance to start there.

Romanced LaeZel early in Act1 but broke soon, then Karly in Act1 and 2
I'd never had SH tell something even a bit "romantic" even with high affinity, no idea of what I should have done or why it don't trigger/how to trigger it

I think that if you're in a romance with two different characters in the early acts, the ability to become romantic with other characters gets blocked off - stuff just won't trigger anymore until you're no longer double dealing. So when you were romancing Lae'zel and Karlach, Shadowheart's party scene and the stuff with the night orchid just wouldn't have been there.

And yeah, as the romances deepen in Act 2, you have to pick one. I don't think you can start a new romance in Act 3 (apart from ones that are meant to trigger then, like Halsin) - by then the origin characters are set.

Ok, weird design choices for a "modern" morals game
I mean even IRL I had romances in my Act3 after several in previous acts lol

For anyone who wants a simple and clear answer go to rivington rest a few times to trigger other scenes (assuming that might be the case). Make sure you dont have any time constraining quests like murder tribunal or when youre entering temple of bhaal so you dont f them up. eventually she will have a exclimation atop of her head. the reason you have to play out long rests is because other scenes are waiting to be triggered in the long rest periods. goodluck
