Festering cove (long term)

So i had a saving issue in the beginning of act III my first playthrough (long story short i reset but missed the festering cove this time through).

Spoilers: I know one the chosen serves bahaal and if i remember correctly there was some discussion of that god in the cove albeit a false god.

My question is does any of that tie into/affect the main story or is it pure side content...did i change something by missing it?

You did not miss anything, save a specialized buff if you leave the fish men and their false god alive. Extra damage to bleeding targets. Which needs specialized attacks to happen. Also, the buff is one of those that is lost upon the death of the character. Furthermore it requires the sacrifice of companion to the false god.

Incidentally, its one of the few things that makes the Slayer form of the Durge much better, as it has bleed causing attack.
