Patch #5 No audio
Just updated to patch #5. I have game audio but when its a cutscene or diologe i have no voice diolage. Iv launched in orginal and dx-11

i have the same issue, i can't hear any voices.

Try verifying your game files.

Also Disable any third-party audio-influencing software you might be using, such as Voicemeeter, Dolby Access, Logitech gaming software, check windows audio output settings/properties and make sure that spatial sound format is set to off.

i reinstalled the game twice, deleted the documents larian studios bg3 folder and still nothing. no spatial sounds no sound apps no nothing.

Worked for me. After a couple dedicated hours to trouble shooting. Ty, I have been playing with an audio bug for so long It was almost a feature of the game XD. Hope to see more great feedback / developer involvment with the community.

I have the same issue. The audio types in the settings, Speakers, TV, Speakers change nothing.. No NPC cutsecene dialog is audible, very faint and broken.. The dialogue issue here is unlike anything any triple A developer charging the $59.99 price point should be telling you, to disable software or profile based programs associated with your headset. For instance I'm using an Asus Strix 7.1 full range headset.. With 5 separate drivers for each ear. Real 7.1, with an external sound control box with built in audio profiles. Going to my desktops 2.1 stereo external sound I can hear everything. So it's incompatibility with 7.1 Dolby, etc.. The center (dialogue) channels are probably coded at a low level, and nothing we can change as they are faintly audible when I crank everything up on the headset..

This is simply them pushing off a real issue while in early access. Don't blame it on peoples hardware/software with something so basically necessary for the enjoyment of the game..

Weird, because unless I have my spatial sound format set to "Windows Sonic for Headphones" I do not get any dialogue though I do get music, sfx and ambient. Just nothing voiced.

turning off third party software and the like is standard tech support procedure. Reduce the number of variables at play and gradually reintroduce them until the issue returns and then you know what (if any) third party software has some sort of conflict.
