I laughed so hard because I made a meme character in my first run.
You start seeing hints of what is going on though if you keep using it.
Ah okay.
Most likely it will affect the story and ending, aka you either change or don't (most likely the ONLY situation in which you won't change / die will be if you should NEVER use its power, but it will have some super rare achieves for it).
I expect that the temptation to use the power will grow over time (dialogue wise), and the powers that it grants over-time to be quite extreme compared to not having them.
There will most likely be some critical element of 'all of the endings' that doesn't change...except it does in this one unique ending, where essentially some master element of the Illithid / Bhaalspawn Plans are undone.
So the tadpole has a penis? I guess my character is gay now.

Tadpole free runs will be common. All that is required is killing everyone you meet. Have you met some of the people in these forums. Can you imagine them not murderhoboing their path across the game?
I imagine them saying "what's a tadpole? Is it a thing in those cut scenes I skip by hitting attack?"
