I'm playing on a PC. Is this perhaps an, ahem, platform issue?

It's happened to me multiple times on separate runs, both the small elevator and the larger circular elevator in the Shar Temple will kill your character despite them being on the platform. Reload and it works fine but it did happen in one of my honor mode runs and I had to use Withers to revive the entire party.
It is just lag. Not an actual game issue.

Yep. That is why it suffices if one just wait on the elevator before triggering them.
Or why the issue does not occur if you make a savegame before triggering the elevator, as the time it takes to do so allows the desynchronisation to catch up.

Elevator platforms without railings = not good. Take the 'slingvator' one at a time.
If you are having issues getting all four to stand on a elevator, just ungroup the ones which are not and then move them.

Users with good PCs do not get such issues lightly.
They would have to reduce their RAM for the game significantly.
