For example, you can have sex with Lae'Zel in Act 1, and spend the night with Shadowheart in Act 1. But in Act 2, Lae'Zel wants you to fight with her. If you refuse, the Lae'Zel romance is over and you won't get the Act 3 sunrise scene. If you accept the fight with Lae'Zel, the Shadowheart romance is over, and you won't get the Act 3 night swimming scene.
Just sex is not romance. You can have sex with at least Lae'zel and Karlach. And others in the game. Just don't make a commitment.
Romance is about loving someone and being devoted to them. So when you both get very old and can no longer have sex for various reasons, you can still have a loving and romantic relationship. Too many people marry and love is not involved. It's lust. Lust in time will fade. But love is forever.
It's fairly common. In RL. I once read a article that about 60% of men have a extra marital affair and 40% of women do. Is there consequences and risks involved? Absolutely. But some manage not to get caught and some fail. Those who fail run the risk of divorce or getting shot.
There is also the risk of spreading a disease or pregnancy if one does not use protection.
