Uhm. Yeah. Game over.
There are certain things which can kill your playthrough. Gale is one of them.
And that is one of the exception events where it kills your playthrough.
So either reload a savegame, or start anew.
Not a bug.
No. It is game over. Working as intended.
And with Gale there tends to be a slight problem (boom!) if he dies. Although, I've never had him die in Act 2 after his issue is contained by outside means, so I'm not sure if the boom! happens or not.
