Nettie's Poison and Gale
I was playing as Gale earlier and when Nettie offered me poison for me to kill myself before I turn into a Mindflayer I was just thinking that Gale would never do that because everyone within a 10 mile radius would just explode and die lol

So he either needs to swear to Nettie that he'll take the poison before he turns or fight her.

Obviously swearing to take it is 100% a lie for Gale specifically. Yet there's no Deception check or any other unique line of dialogue for him in that sequence and I just found that strange.

Yeah, that's an oversight. Obviously every other character including custom ones can mean it (at least in the moment, whether they'd go through with it in the end is another question). The writers were bound to miss a few odds and ends like that.
Makes me wonder; if Gale was taken over and a new Mind Flayer born with the orb in it I wonder what either the Chosen or the Netherbrain would do with it.
Orin at least would want to find the most populated spot she could (Waterdeep probably) and set it off for the glory of Bhaal.

clearly blowing up would be bad, but would it be worst than being a mindflayer with his condition? considering he is also carrying a scroll of true Resurrection, he could believe that temporary death is better than illitid.

Not sure that'd help. True Resurrection reverses any wounds or fatal conditions that existed before death but ceremorphis doesn't count as either, so he'd just be restored to life and then resume his transformation.
Also depending where he was blowing up would be much worse; anywhere near a city and he'd kill way more than he would as a hungry illithid.

Isn't your characters tampering with the corpse the cause of the worm to leave the body.

I know independent tadpoles become separate horrors. These cases are pre-insertion. Neothelid and Illithocytes. I'm not sure anyone on the Sword Coast has a technique to remove the tadpoles beyond the inserted 'True Souls' telepathically calling up each other.s worm.
