Non-invasive procedure achievement bug?

It's been a couple of days that I try to get this achievement, but I can't. Is it bugged, or am I missing something?

Let's give you some details: first I convinced all the nurses to kill themselves, and then I engaged combat with Malus Thorm. He tries to revive the nurses many times during combat, to get their weapons and proceed with the surgery. To avoid this, I tried the following strategies:

- N1: using a combination of Hunger of Hadar and Evard's Black Tentacles, cast by my warlock and wizard, to block the surgeon in place and instantly kill the nurses as soon as they spawn. This didn't work to obtain the achievement, even though Malus didn't perform surgery.

- N2: I thought the problem was that trying to revive the nurses doesn't give Malus the opportunity to attack me, so I tried using Goading Attack to attract him towards Lae'zel. He didn't revive any nurse. He successfully attacked my character, but I got no achievement.

- N3: At this point, I also thought about the fact that using Hunger of Hadar and Evard's Black Tentacles was killing the poor guy that was being tortured by Malus, and maybe his death was the cause of all my problems. I adopted the strategy N2 again, but this time without those spells or other ranged attacks. The fella survived. No achievement.

he apparently needs a nurse alive to attempt surgery on you, I saw someone else with this problem and they fixed it by keeping one nurse alive and having malus ready the surgery attack then killed him
