Battle-Worn Blade[bg3.wiki] is a rusted piece of equipment found in Zevlor's chest at act 1. It has the following description:

"A fine but well-used sword. It seemed to have once belonged to a holy order, but the indication of rank and patron deity at the hilt have recently been filed down"

Every time i found it i kept to see if the sword would be restored later, to no avail. The feeling i have with the sword was the same with the Shattered Blade of Aihonen[icewinddale.fandom.com] from icewind dale, that later can be upgraded in quest to become the Restored Blade of Aihonen +5 [icewinddale.fandom.com], and can even be further upgraded in that game expansion Singing Blade of Aihonen +5 [icewinddale.fandom.com].
