Do people joining your game auto-download whatever mods you have?
Not 3rd party ones obviously, but any in-game mods, do they have to manually get them or is the game smart enough to grab what's needed to play?

That's false. Its absolutely viable and possible. Just not something they want to do, they want to move on to a new game. Which is fair, this one is finished.

I know it's a different system, but it happens in Larian's other game Divinity 2, though it's though Steam Workshop. I imagine same fundamentals when it comes to the "privacy structures" however. I can see them adding it in a small update, because there'll still be some updates to BG3, just no more major updates.

No, it is not considering it is a P2P game.

they do that in dos2 but the auto download time is absurd. i am not sure about this game
