She will be in act 3 if she survives.
Alfira survives the DURGE incident but loses her voice as a result, now she can be a permanent companion and it makes sense that she doesn't have any dialogue about the rest of the story going on :D
Because they felt it so important to the Dark Urge storyline to have him/her kill an innocent bard, they wrote in a backup just in case something had made Alfira unavailable. I highly doubt they had intended this as a way to "save Alfira", but once it was discovered, people used it.
For that matter, it's pretty clear they never intended for Minthara to be able to survive if you kill the goblins, but people discovered ways around that.
The devs eventually legitimized the whole "recruit Minthara" thing, but it seems like they were not interested in doing the same for Alfira.
The devs already explained that she is not a companion.
