I enjoy it as a shake up to gameplay and a way to make some less important fights more fun. Not so useful for serious fights.

I went from 60% hit chance without form down to 25% hit chance in form.

Personally I do not think that balancing it is an issue. It can be unbalanced in gameplay, so much is fine as long as the moral consequence is harsh enough.
Absolutely agreed. Basically if you're playing a martial character or martial multiclass that can stand up to being a front-liner then this form is pointless at best or a downgrade at worst (depending on build and gear). For a squishy it can be a literal life-saver, especially in the big duel that happens at the culmination the Urge's story arc.
