Unfortunately, I think this is the problem in a nutshell. Initiative in honor mode is everything, especially for this fight. Every character has to have the Alert feat, or use elixir of vigilance.
If you did that and she still went before everyone in your party, that's terrible luck.
Calm Emotions spell can add an extra layer of insurance in case she still turns first. You can cast it before walking down to her.
Either one-round her or stop all the chanters in round one to disable Edict of Bhaal.
Bhaal's edict is only applied if you don't do a melee attack on Orin.
No better use for your 6th-level spell slot. (You can even get the spell slot back again if you have the spellcrux amulet from the Warden in Act 2.)
hero feast is very good spell, my lvl12 cleric always cast lvl 6 aids and lvl 6 hero feast every single long rest (two staff that provide arcane battery).
leave me level 6 spell slots for stuff like heal/sunbeam/mass cure wound
that's always the part of high level DND, from tier 2 onward lot of monster can also do big aoe or crowd control ability. but then player side should have good amount of counter measure too. unless you play no magic party, pretty much all spell casting class should have some ability to help out against it
also improve your saving throw helps a lot, any class with wisdom saving throw bonus also help against it. if you have paladin with a +5 charisma. the aura of protection give you plus 5 bonus, that's 25% increase of defend against it on saving throw
a lvl10 paladin with aura of courage will allow your allies immune to fear as well
1) brute force:
* use something like magic missle to remove all of Orin's Unstoppable and whole party unload everything they have on her. all of your smite, level 6 spell slots...etc just unload everything. should be able to kill her in one turn. use haste potion so you can do one extra spell attack or attack...etc
2) stop the unstoppable:
* as long as her cultists inside the sanctuary circle alive, she get to refresh all of her unstoppable every single turn. use spell like black hole group all those cultists together and drop all AOE on them. some stuff like arrow of many target also can bounce on them too.
once those cultists died, you are free to take your time to kill orin. you can try to use banishment or some spell to send her away while dealing with cultists
