Mods to make combat Real-Time?
Are there any settings or mods that change the combat system to something similar to games like Pillars of Eternity, with real-time combat that doesn't require pausing and waiting for enemies to act? I really want to get into Baldur's Gate, and I'm also into games like Neverwinter and others, but the combat here is holding me back.

No. People have tried with Larian's other games like DOSII but they always fail for 2 reasons:

1: lack of interest.

2: Even the modders who tried have admitted that a mod like that would require rebuilding the entire game from scratch.

It wouldn't really work if you think about it. Maybe if you solo'ed one character, but how are you going to activate bonus actions and reacts and target spells for 4 characters in real time? Without a pause button I might add, but even if you had one you would have to be pausing so much for all the abilities that you might as well just leave it turn based.

D&D 5e just isn't going to work with it. It worked decent in BG12 and 2 since there wasn't that many abilities and no such things as reacts or bonus actions in AD&D

BG1 and 2 feels pretty dated at this point, to be honest.

I've also tried Icewind Dale, but gave up after a while because of having to read all the dialogue.

I forced myself to play BG1 and 2 a long time ago as I was a tabletop D&D player but never liked the real time thing and was a deal breaker honestly. I even held a initial prejudice against BG3 because I was assuming it worked the same way.

It was a big relief when I discovered otherwise and BG3 being turn based is surely one of the points that made it GOTY.

I doubt the companions have any combat AI at all since the player makes all of their actions.

Well, they do since Astarion does do some stuff when you get ambushed in camp by his 'family' in Act 3

But it's another thing entirely to have them perform reliably throughout the entire game with as large an ability and option set your average player character has.

There's at least one other case where a companion can enter combat on her own. She defaulted to using the most damaging attacks she could do, which meant throwing all the smokepowder bombs she had on her.

The validity of the GOTY was disputed since LoU2. We've seen political discourse dictate GOTY, therefore, in my eyes it's all invalid.
