Teachings of Loss: The Nightsinger missing NOT A BUG, you did something else

I ran into this "bug" myself on my first playthrough and I see there's a lot of discussion around it. No it is not a bug: THE BOOK IS DESTROYABLE.

The book is a simple 2HP object. If you have any characters or summons that fire AoE attacks, it is exceptionally easy to hit the book as well as the Justiciar that is typically found next to it. One shot with virtually anything will destroy that book, LOCKING YOU OUT OF COMPLETING THAT PUZZLE, so watch your fire!

I remember a comment along these lines. This seems to be a perfectly reasonable explanation. Thank you.

I have the book and placed it, but it says "Ingredients Missing" after placing it.

Ran into this issue myself on my first try of that library, it was the Burning Cinder blasts of my fire elemental that buggered me. After reloading a save I went overkill in prep and put an Invisible melee fighter right next to the bookcase before starting the fight, and nabbed the book before I had anyone firing anything serious.

Did you click the button at the bottom of the popup to complete the placement step?
