Question though - I broke up with Karlach to be with Minthara and now have spurned Minthara. Is there a way to romance Shadowheart in Act 3 after completing her quest? Will there be an option available?

Nope, you have to set up relationships with origin characters (That being Astarion, Gale, Karlach, Lae'zel, Wyll and Shadowheart) in act 1 and if you are romancing two of them in act 1, you will have to make a choice when their romance progresses and you get the next romantic scene with either of them.
