Her disappearing on death with a *poof* could be one of the old 'good recruitment' mods for her, in which case there might not actually be a problem. If that's the case, you should be able to recruit her fine in Act 2. Is there a smoke effect when she dies?
Posting your mod list would help, but you really shouldn't be enabling or disabling mods on an existing playthrough.
That's your issue I bet. I just fought her last night and knocked her out and she stayed that way. Came back later and she was gone. Which is normal. I don't use any mods.
It is however possible to recruit her and save the grove after a recent patch, you recruit her later when you save her from the tower in act 2.
The poof is from the Daughter of Lolth Good Recruitment mod. I still use it in my runs. She disappears in a puff of smoke.
Download the console version of the mod and you can see in the logs whether it worked or not.
