What needs to be done for this? Do I need to be on good terms with Karlach or can I do this later via a dice roll? Do I need to have her in the group to offer her this? Will the Emperor remain alive and will it be possible to free Orpheus?

You need to have Karlach in your party to have the option for her to become Illithid. If you free Orpheus the emperor will flake and oppose you by joining the elder brain.

After you free Orpheus - you have to do that - you get the usual "who's gonna become an illithid - you or me?" dialogue. Say you need to discuss it with your party, then in turn have Karlach have the dialogue with Orpheus, and volunteer herself to become illithid. She's willing, as, after all, it is an option to save her life.

Это на самом деле связанные, но разные ветки развития сюжета. Вопрос с Императором ты решаешь по сути параллельно. Есть варианты освободить Орфея (Император обидится и свинтит, и тогда его придется валить в конце), и предложить Карлах стать иллитидом (в этом случае ее преобразование совершит Орфей), или же предложить Карлах стать иллитидом вместо ГГ сразу, тогда Император останется, триггернет ее преобразование сам и позволит ей сожрать Орфея
